29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a- 600 Route 38 East, Maple Shade NJ: An employee, wearing a full body harness without a lanyard attached, was exposed to fall hazards when working from an elevated man lift basket that was attached to the forks of a Terex Model GiroLift 3514. Condition observed on or about 10/21/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.
