29 CFR 1926.403(e): All splices and joints and free ends of conductors were not covered with insulation equivalent to that of the conductors or with an insulating device designed for the purpose:    (a) Boiler Room Entrance - Employees using a yellow type extension cord were exposed to electrical hazards when the extension cord was spliced and covered with tape not designed to act as an insulating device.
