OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees: a) Missouri American Water @ Clayton Road & McKnight Road Intersection, St. Louis, MO 63124: On or about October 22, 2014, the employer did not ensure that employees who conducted waterline work in an excavation on a closed westbound lane of Clayton Road, immediately after the Clayton Road and McKnight Road intersection were protected against struck-by hazards related to vehicular traffic. Employees held out their hand to control oncoming traffic to allow equipment to clear an active intersection. b) Missouri American Water @ Clayton Road & McKnight Road Intersection, St. Louis, MO 63124: On or about October 22, 2014, the employer did not ensure that employees who conducted waterline work in an excavation on a closed westbound lane of Clayton Road, immediately after the Clayton Road and McKnight Road intersection were protected against struck-by hazards related to vehicular traffic. The employer did not ensure that advanced warning signs were in place for motorist who traveled on northbound McKnight Road toward the Clayton Road and McKnight Road intersection. Abatement Note: Among other methods, recognized and feasible means of abatement to correct this hazard is to: 1. Follow the requirements set forth in Part 6, chapters 6E (Flagger Control) and 6F (Temporary Traffic Control Devices) of the U.S. Department of Transportation (DOT) Manual on Uniform Traffic Control Devices (MUTCD). 2. Provide and enforce the use of advanced warning signs to alert traffic. 3. Provide and enforce the use of flaggers, using hand signaling devices such as flags, or stop/slow paddles. 4. Provide employees with appropriate training and information on company policies and the proper use of equipment.
