29 CFR 1910.23(a)(8)(ii): Floor holes into which persons can accidentally walk were not guarded by floor hole covers of standard strength and construction. While the covers were not in place, the floor holes were not constantly attended by someone or protected by removable standard railings: Chlorine Room: Employees were exposed to tripping hazards where 16 different holes which measured approximately 5 1/2 inches in their least dimensions were not covered, were not protected by railings, or were not constantly attended by someone.
