29 CFR 1910.23(a)(8)(ii): Floor holes into which persons can accidentally walk were not guarded by floor hole covers of standard strength and construction. While the covers were not in place, the floor holes were not constantly attended by someone or protected by removable standard railings:     Chlorine Room: Employees were exposed to tripping hazards where 16 different holes which measured approximately 5 1/2 inches in their least dimensions were not covered, were not protected by railings, or were not constantly attended by someone.

29 CFR 1910.119(j)(1)(ii): Piping systems including piping components such as valves were not labeled with the materials running through the piping systems:        Evaporator Room: A piping system carrying pressurized chlorine gas was not labeled with its contents to prevent unintended release of chlorine and consequent injuries to employees.       29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:     Evaporator Room: A piping system containing hazardous chemical (pressurized chlorine gas) was not labeled with its contents.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before further use:        A/B Chlorine Injector Room: At the time of inspection bolts on a Pureflex valve were loose and water could be seen coming out of the loosely packed valve indicating deficiencies in maintenance of the Chlorine Injector systems.

29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed:   Chlorine Room: Hoisting  mechanisms of a " ton crane" used for a ton container of chlorine cylinders were not inspected frequently and certified as required by the standard and as noted under "CP-C4 Monthly Equipment List Lines 37 and 38," exposing employees to struck-by hazards.
