29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about October 23, 2014 at a job site located at 1130 Emanuel Cleaver Blvd in Kansas City, MO- employees engaged in commercial roofing activities on a low-sloped roof with unprotected sides and edges were exposed to serious fall hazards, in that there was no Warning System in use and employer did not employ the use of a Safety Monitor for workers at the edge of the roof. As result, employees were exposed to fall hazards up approx. 23 feet above the surface below. Abatement certification is not required for this item.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall: On or about October 23, 2014 at a jobsite located at 1130 Emanuel Cleaver Blvd in Kansas City, MO- One employee was exposed to serious fall hazards, in that he was observed carrying roofing materials in one hand while climbing a 15 ft ladder to the roof work area. Abatement certification for this violation is not required.
