(a) Work Bay: On or about October 22, 2014 and times prior thereto, compressed air nozzle was operating at 120 p.s.i.. and would not reduce to below 30 p.s.i. if dead-ended. This exposed employees to potential air embolism or laceration to the skin if nozzle came in contact with there skin.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. (a) Mezzanine: On or about October 23, 2014 and times prior thereto, the circuit breaker panel did not have a cover installed. This exposed employees to electrical shock hazard from the 15 amp breakers. (b) Mezzanine: On or about October 23, 2014 and times prior thereto, panel 316 had an opening that expose live electrical parts. This exposed employees to potential electrical shock hazard if they contacted conductor. (c) Floating shop: On or about October 23, and times prior thereto, there was an opening on front side of breaker panel exposing the conductor. This exposed employees to potential shock hazard from incidental contact.
