29 CFR 1926.403(b)(2): 29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about October 23, 2014; on the west side of the jobsite, the employer allowed employees to utilize extension cords in a manner that was not in accordance with instructions included in the labeling. Employees connected electrical extension cords together to create a chain of two (2) extension cords and one (1) three-way short cord in order to power a hand-held reciprocating saw (Dewalt-DW304P).

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about October 23, 2014; on the west side of the jobsite, the employer allowed employees to utilize an extension cord with inadequate strain relief in order to power a hand-held reciprocating saw (Dewalt-DW304P). Employees used the reciprocating saw to cut and remove unneeded wooded frame work.
