29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about August 15, 2017, at Cecilia Branch Library, the employer failed to ensure that equipment was installed and used in accordance with instructions for use. A household use power strip, New Point Surge Protector, household use, was used in the manner in which it was not intended. The surge protector was being used as a power source for two DeWalt battery chargers and a DeWalt circular saw.

29 CFR 1926.405(g)(1)(iii)(C): Flexible cords and cables were run through doorways, windows, or similar openings: On or about August 15, 2017, at Cecilia Branch Library, the employer allowed employees to run flexible cords through doorways. An employee used the extension cord as a power source for a household power strip which was used to plug in a DeWalt Circular saw and two DeWalt battery chargers.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about August 15, 2017, at Cecilia Branch Library, the employer failed to ensure that extension cords had adequate strain relief. Employees used the extension cord as a power source for a household power strip. The power strip was used as a power source for two DeWalt battery chargers and a DeWalt circular saw. The P & T Construction Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.405(g)(2)(iv), which was contained in OSHA inspection number 1003337, citation number 1, item number 2 and was affirmed as a final order on December 5, 2015, with respect to a workplace located at 4328 Ryan Street, Lake Charles, LA.
