29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment: a.) Facility - On or about 10/23/14, the employer had not performed inspections and tests on chlorine process equipment, chlorine tank location #8, including Vacuum Regulator Pressure Control (VRPC), flex line, and diaphragm. Among other feasible abatement methods include but are not limited to following: 1) applicable manufacturer's recommendations for inspection; 2) established recognized engineering practices (such as the Chlorine Institute); 3) prior operating experience; 4) technical association guidelines; and/or 5) legal requirements. ABATEMENT CERTIFICATION REQUIRED

Facility - On or about 10/23/14, the employer did not document each inspection and/or test that was performed on process equipment wherein the document identified the date of inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test. The employer was missing inspection and test records from chlorine process equipment, at chlorine cylinder location #8, including Vacuum Regulator Pressure Control (VRPC), the flex line, the diaphragm. ABATEMENT CERTIFICATION REQUIRED
