OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a hazard of being thrown off of a stand-up type forklift.     A) Shipping Department, on or about October 27, 2014, employees operate Crown stand-up type forklifts with 3 wheels in side tractor trailers/containers without determining the floor load capacity of the trailers/containers before operating the forklift inside the trailer/containers, creating a condition where one of the wheels could fall through the floor of the trailer causing the operator to be dislodged from the forklift operator's station.      One feasible and acceptable abatement method to correct this hazard is to determine the floor load capacity of the trailer and compare it to the weight of the forklift per wheel with and without a load and do not over load the floor capacity of the trailer.
