Juan Diaz employees located at front, right of building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA; employees were exposed to fall hazards: a. of up to 14 feet where at the front, right of the building, Diaz employees were observed climbing the frame of the scaffold in lieu of installing a ladder. b. of up to 7 feet where at the rear, left of the building, Greenway employees were observed working from a scaffold that had no ladder installed.

On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA; employees were exposed to struck-by hazards where the working levels had materials such as hand tools and bricks on them: a. 14 foot scaffold where EIFS operations were performed by MMC Construction located at the front, left of the building. b. 14 foot scaffold where brick laying operations were performed by Juan Diaz located at the front, right of the building.
