29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about October 29, 2014, employees were exposed to a fall hazard greater than 6 feet in height to unpaved ground level below while installing wood decking on a rooftop without having fall protection measures in place.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:  On or about October 29, 2014, employees were exposed to a fall hazard of greater than 6 feet in height to unpaved ground level below and were not being trained to recognize the hazards while installing wood decking on a rooftop without having any fall protection measures in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about October 29 , 2014, an employee was exposed to fall hazards greater than 6 feet in height to unpaved ground level below when using a wooden job made ladder having less than 3 feet of ladder sections above the landing, while climbing the ladder for access and egress to and from first level landing.
