29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: On or about October 30, 2014, at the above addressed site, employees were exposed to a fall hazard while using an aluminum extension ladder set up in an active driveway that was not secured to prevent accidental displacement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 30, 2014, at the above addressed site, employees were exposed to a fall hazard of approximately 15 feet 10 inches when performing roof work on a low slope roof while not being protected from falling to a lower level. MILLENNIUM MAKEOVER, INC.WAS PREVIOSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 768642, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS FINAL ORDER ON 01/28/2013, WITH RESPECT TO A WORKPLACE LOCATED AT 1230 SW 2ND AVENUE IN MIAMI, FLORIDA.
