While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.      a)  The 18.5 long by 3.5 foot wide pit with 10 by 10 inch openings in the germination room on floor #4 in malt house #3 was not protected on all sides to prevent employees from falling into the space during transfer of grain between the germination operations.
