29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Bander #3- Employees cleaning rollers and rails with rags and a scraper are not protected from amputation and crushing injuries where non-interlocked barrier guards are bypassed and the machine is not locked out. On or about October 23, 2014 and employee sustained a finger avulsion when cleaning rollers with a rag and scraper while the bander was operating. This cleaning operation does not meet the requirements of the control of hazardous energy (lockout/tagout) exception because the work is not performed using alternative measures which provide effective protection.
