29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  A)    Rubber Recycling Technology, Inc., located at 441 Grand Lake Road, Celina, OH 45822: On or about September 30, 2014, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of equipment such as, but not limited to, big rubber guillotine (custom-made in the facility). Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic while performing maintenance activities such as, but not limited to, changing blades. Employees were exposed to amputation hazards.    B)     Rubber Recycling Technology, Inc., located at 441 Grand Lake Road, Celina, OH 45822: On or about October 9, 2014, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of equipment such as, but not limited to, the Farrel Foundry rubber mill. Employees did not control all hazardous energy sources such as, but not limited to, electrical, water pressure, and heat while performing maintenance activities such as, but not limited to, replacing t-union and water hose. Employees were exposed to struck-by and heat hazards.

29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage:   A)    Rubber Recycling Technology, Inc., located at 441 Grand Lake Road, Celina, OH 45822: On or about September 30, 2014, the employer did not ensure that big rubber guillotine (custom-made in the facility) was properly turned off/shut down while employees conducted activities such as, but not limited to, changing blades. Employees were exposed to injuries from unexpected start-up of the machine and energy release.    B)     Rubber Recycling Technology, Inc., located at 441 Grand Lake Road, Celina, OH 45822: On or about October 9, 2014, the employer did not ensure that the Farrel Foundry rubber mill was properly turned off/shut down while employees conducted activities such as, but not limited to, replacing t-union and water hose. 29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  A)    Rubber Recycling Technology, Inc., located at 441 Grand Lake Road, Celina, OH 45822: On or about October 9, 2014, the employer failed to properly train employees to lockout/tagout equipment such as, but not limited to, big rubber guillotine (custom-made in the facility) and the Farrel Foundry rubber mill before employees conducted activities such as, but not limited to, replacing blades on the guillotine and parts on the rubber mill. Employees were exposed to amputation, struck-by, and heat hazards while all hazardous energy sources were not controlled prior to conducting work.
