29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a.) Rubber Recycling Technology, Inc. located at 441 Grand Lake Road Celina, OH 45822; On or about May 2, 2016, the employer did not assure the Farrell Rubber Mill was properly deenergized before standing on the conveyor to push the rubber over the mill. Rubber Recycling Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(2), which was contained in OSHA inspection number 1005219, citation number 1, item number 1b and was affirmed as a final order on April 10, 2015, with respect to a workplace located at 441 Grand Lake Rd. Celina, OH 45822.
