29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) Production Area Near Winders : Piles of plastic scrap and product was allowed to build creating a slip hazard on or about January 14, 2015. b) Production Area Near Contractor Bags & Compactor: A pile of plastic scrap was allowed to build creating a slip hazard on or about January 14, 2015. c) Production Area, Extruder lines: Electrical cords and hoses created trip hazards on the floors and walkways, on or about January 14, 2015. d) Production Area, Extruder line 6: Grease, and pellets created slip and trip hazards on the floors, on or about January 14, 2015. e) Employee Break Room: Water on the floor created a slip hazard on or about January 14, 2015.
