29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) Production Area, First Floor: At floor of exit access near stairway accessing the second floor, loose pellets created slip hazards, on or about June 15, 2015. b) Warehouse Area, Walkway Near Machine 8: A puddle of water was allowed to remain on the floor, creating a slip hazard on or about June 15 2015. c) Shipping Area: Water was allowed to remain on the floor, creating a slip hazard on or about June 15, 2015. Film Pak Extrusion LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(2), which was contained in OSHA inspection number 1005293, citation number 1, item number1b and was affirmed as a final order on April 13, 2015 with respect to a workplace located at 99 5th Avenue, Paterson, NJ 07524.
