On or about October 29, 2014, at BJ Brew House & Restaurant, the employer did not ensure that the scaffold system was tied into the structure as recommended by the manufacturer. Employees were installing thin brick from the scaffold system that was approximately 28 feet in height.

On or about October 29, 2014, at BJ Brew House & Restaurant, the employer did not ensure that the scaffold system was complete with a means of access and egress. Employees gained access to the scaffold by means of the metal frame.

On or about October 29, 2014, at BJ Brew House & Restaurant, the employer did not ensure that a guardrail system was installed on all open sides and ends of the scaffold. Employees were using the scaffold to install thin brick to the corner of the building. Instance 1) On the fourth tier the guardrail did not extend the full length of the scaffold. Instance 2) On the third tier there was no guardrail on the open end of the scaffold. The existing position of the guardrail system was not adequate.

The scaffold had three planks where 4 were required. Employees were using the scaffold to install thin brick. The World of Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451 (b)(1), which was contained in OSHA inspection number 314769282, citation number 1, item number 1 and was affirmed as a final order on April 1, 2011, with respect to a workplace located at 5001 Pennical Parkway, Covington, LA.
