29 CFR 1910.147(c)(7):     Training and communication.    (a) On or about October 29th, 2014, and at certain times before, employees working in the fabrication department and operating the YLM pipe bending machine (company machine number F407) were exposed to possible crushing-type injuries and possible amputation hazards in that the pressure-sensitive safety mat in use with the machine was not large enough to prevent employees from having any parts of their bodies in the danger zones during the machine's clamping and bending operating cycles.      On October 29th, 2014, a worker sustained a serious injury when the subject YLM machine executed a clamping cycle and entrapped the worker's arm.        Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
