29 CFR 1910.253(a)(4): Workmen in charge of the oxygen or fuel gas supply equipment, including generators, and oxygen or fuel gas distribution piping systems were not instructed and judged competent by their employers for this important work before being left in charge: On or about November 03, 2014, the employer did not ensure supervisors were instructed and judged competent regarding installation of flashback arrestors for oxygen-fuel gas cutting operations. Employees were exposed to burn hazards.
