29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: A.) Frontier General Contracting, Inc.; Cathedral Prep Event Center - On or about 10/27/14 and at times prior thereto, Subcontractor's employees were in an excavation conducting work activities that included, but were not limited to, constructing a masonry wall. The spoil pile was at the edge of the excavation while employees were in the excavation performing work activities. The spoil pile was greater than 10-feet in height.

29 CFR 1926.652(c)(4)(ii): Designs were not in written form and did not meet the requirements listed in 1926.652 (c)(4)(ii) (A) through (c)(4)(ii)(B): A. Frontier General Contracting, Inc.; Cathedral Prep Event Center - On or about 10/27/14 and at times prior thereto, a steel plate system was installed as a protective system for the excavation cut into the south side of the job site (i.e. side of the job site adjacent to the football field). Subcontractors were performing work activities below the steel plate system and wall of the excavation. The design of the steel plate system was not onsite and in written form. In addition, specifications and tabulated data for the steel plate system were not established and maintained on the job site in written form.
