29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  Bill Hawk dba Hawk Construction had five employees exposed to struck-by hazards in the eyes and fall hazards, on a residential construction site in Tuscola, Illinois.  The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employees activities.  Bill Hawk dba Hawk Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.20(b)(1), which was contained in OSHA inspection number: 281625, citation number 1, item number 1a and was affirmed as a final order on June 19, 2012, with respect to a workplace located at corner of Rt. 36 and 800 E., Tuscola, IL.

The employer did not have a competent person at the construction site conducting frequent and periodic safety inspections at the construction site.  Bill Hawk dba Hawk Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.20(b)(2), which was contained in OSHA inspection number: 281625, citation number 1, item number 1b and was affirmed as a final order on June 19, 2012, with respect to a workplace located at corner of Rt. 36 and 800 E., Tuscola, IL.
