Abatement certification was not received for the following violations:      a)    Citation 1 Item 1 and  b)    Citation 3 Item 1.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    Employees engaged in roofing activities were exposed to fall hazards of approximately of approximately 13ft. and 18ft. in Tuscola, Illinois and the employer did not ensure employees were adequately using fall protection.         Bill Hawk dba Hawk Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1005998, citation number 2, item number 2 and was affirmed as a final order on July 23, 2015, with respect to a workplace located at 201 Forest Glen, Tuscola, IL 61953.   Bill Hawk dba Hawk Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 281625, citation number 1, item number 2a and was affirmed as a final order on June 19, 2012, with respect to a workplace located at corner of Rt. 36 and 800 E., Tuscola, IL.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:     The employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards.  There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
