Rich Company Incorporated - The employer failed to adequately develop, document and utilize energy control procedures for their employees who serviced or performed maintenance on hazardous equipment [e.g Frame Cutting Machine, Saw Optimizer machine, Foam Operation Machines] within the facility. This was observed on October 30, 2014.

Rich Company Incorporated - The employer did not ensure that an adequate inspection of the propane powered Hyster industrial trucks [e.g. models S89XL and S50FT] were conducted prior to being operated by the employees in the production facility.  This was observed on October 30, 2014.

29 CFR 1910.242(b):     Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    a) B.F.Rich Company Incorporated. - Several Employees are exposed to eye injuries and/or skin embolism when operating compressed air nozzles. Employees were operating the equipment to clean the surfaces of their work areas and themselves which  were identified without restrictor valves. The compress air nozzles being operated were measured releasing a stream of air pressure between approximately 40 to 80 P.S.I. This was observed on October 30, 2014.

Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either:    a) B.F. 29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a)  B.F. Rich Company Incorporated. - Employee who handle hazardous chemical products [e.g. Xylene, Aliphatic and Aromatic Hydrocarbons], were not provided training on the new labels and safety data sheet format which was instituted in December of 2013. This was observed on October 30, 2014.
