29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      On 29 Oct 2014, Employees of Innovative Construction & Roofing LLC, were observed conducting roofing work activities located at 35 Loren Woods Drive, Saint Louis, MO 63124, Three employees were working on a steep roof with a 6/12 pitch slope with a working area height of 20- 28 feet.          Innovative Construction & Roofing, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 314470394, citation number 1, item number 2, and was affirmed as a final order on 5/26/2011, with respect to a workplace located at 2901 N.W. Aspen, Blue Springs, MO 64015.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    On 29 Oct 2014, three employees of Innovative Construction & Roofing LLC, were observed working on a residential site located at 35 Loren Woods Drive, Saint Louis, MO. All three employees were exposed to potential fall hazards by the use of the access ladder that was not at least 3 feet above the upper landing surface to the roof that had a 6/12 pitch steep slope and a working area height of 20 feet ground to eave and 28 feet ground to peak.      Innovative Construction & Roofing, LLC. was previously cited two previous times:   1) for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 314470394, citation number 1, item number 3, and was affirmed as a final order on 5/28/2011, with respect to a workplace located at 2901 N.W. Aspen, Blue Springs, MO 64015.     Additionally, Innovative Construction & Roofing, LLC. was previously cited   2) for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 314281908, citation number 1, item number 3, and was affirmed as a final order on 4/09/2010, with respect to a workplace located at 823 Blossom Lane, Saint Louis, MO 63119.
