29 CFR 1926.20(b)(2):  Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On 12/09/2015, the subcontractor for Innovative Construction & Roofing LLC, was installing a roofing membrane on the roof of a commercial building located at 401 Mar-Le Drive, Wentzville MO. Two foremen from Innovative Construction & Roofing LLC were on the roof at the site while the employees were working.  The foremen did not conduct an adequate inspection of the job site, materials, and equipment.  During the inspection, the subcontractor's employees were exposed to the following hazard:  a)The crew leader and nine employees were repairing the roof, which was approximately ten feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
