29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Sanding Room: The employer did not provide a medical evaluation for employees wearing 3M 5200 half mask respirators when sanding plastic material, on or about 11/5/14. Reiss Manufacturing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 1910.134(e)(1), which was contained in OSHA inspection number 315487165, citation number 1, item number 9 and was affirmed as a final order on 3/23/12 NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
