29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   a) Injection molding area: Employees standing on injection molding machines at a height of approximately eight feet and were not protected from falling to the ground below, on or about 8/4/15.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.    Reiss Manufacturing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.23(c)(1) which was contained in OSHA Inspection Number 315487165, Citation Number 1, Item Number 4 and was affirmed as a final order on 3/23/12 with respect to a workplace located at 75 Mt Vernon Rd, Englishtown NJ 07726.

29 CFR 1910.147(c)(4)(ii)(B): The procedures did not clearly and specifically outline the procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  a) Injection molding area: The lockout tagout procedures for mahcines such as, but not limited to, the Cincinnati Milacron injector, were incomplete. It did not include all of the lockout directions as required by the manufacturer, including, listing of the additional energy sources besides electrical, applying a lock to the air and water supply valves, draining any residual energy and checking the hydraulic pressure, on or about 8/4/15.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
