29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  The employer allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections.  The most recent occurrence was at 866-870 Lionsgate Drive, St. Louis, MO  63130, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with company safety and health policies and OSHA standards.  The workers were exposed to the following hazards at the job site:  a)  A worker was not wearing a protective helmet while exposed to struck-by hazards; b)  Workers were exposed to electrical hazards when the employer failed to ensure that all electrical power equipment (power tools and flexible cord sets) were visually inspected for external defects prior to use; c)  Workers were exposed to fall hazards when working from ladder jack scaffolds that did not have safe access to the working platform; d) Workers were exposed to unplanned collapse hazard when a damaged ladder was used to support one end of a ladder jack scaffold; e) Workers were exposed to fall hazards while performing siding installation from ladder jack scaffolds without personal fall arrest systems when the platform heights were approximately 15-17 feet above ground; f)  Workers were exposed to an amputation hazard while using an inadequately guarded saw.
