29 CFR 1910.23(e)(1): The railing did not consist of top rail, intermediate rail, and posts, and did not have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level:    On or about October 16, 2014 at Building 91, on the modified C141 wing stand, an employee was exposed to a fall hazard of ten feet to the lower level where the top and mid chain rails had more than three inches of deflection and were not taught.    Notice 1, Item 2, 29 CFR 1910.23(e)(1) as required by 29 CFR 1960.8(b) : Standard railings did not consist of a top rail, intermediate rail, and/or posts, and/or did not have a vertical height of 42-inches (106.7 cm) nominal from the upper surface of the top rail to the floor, platform runway, or ramp level.  The employer was cited for a violation of the same standard and same hazard on 01/20/2011, Inspection #314204181, final order/settlement date of 02/15/2011, and means of abatement.

29 CFR 1910.29(a)(3)(v): The work level platform of scaffolds (towers) were not of wood, aluminum, or plywood planking, steel or expanded metal, for the full width of the scaffold, except for necessary openings and work platforms were not secured in place:    On or about October 9, 2014 at Building 91:    a. on the modified C141 maintenance stand, an employee fell ten feet to the lower level where the hinged floor was left opened and a standard railing system was not installed.    b. on the modified C141 maintenance stand, employees are exposed to a fall hazard of approximately ten feet to the lower level where the side of the maintenance stands left a fourteen inch opening next to the aircraft.    c. on the wing of a C-130 Hercules, an employee was exposed to a fall hazard to the lower level when he had to untie one his lanyard to move over a wing hard point.
