Energy control procedures specifically for the HydroMat Machines had not been reviewed nor had any of the companys authorized employees performed periodic inspections.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program were understood by authorized, affected, and other employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by authorized employees:   a) Control Devices, LLC @ 1555 Larkin Williams Road, Fenton, MO 63026: On and before November 5, 2014, machine operators performed service and maintenance work to include, but not limited to machine set-up, unjamming of parts, and tool changes, on the Hydromat Machines. The Hydromat Machines had multiple energy services including hydraulic, electric, and thermal. Operators were not considered authorized employees to conduct lockout tagout. This condition exposed employees to amputation hazards.

29 CFR 1910.178(a)(4): Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly when a modification which affected its capacity was performed:    a) Control Devices, LLC @ 1555 Larkin Williams Road, Fenton, MO 63026: On and before November 5, 2014, the employer installed a 7 foot boom for the Mitsubishi FGC40K forklift which was marked CD001. The altered capacity of the forklift was unknown. Employees utilized the boom for the purpose of lifting and transporting bundles of steel and brass materials in and around the facility. This condition exposed employees to struck-by hazards.

29 CFR 1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle:    a) Control Devices, LLC @ 1555 Larkin Williams Road, Fenton, MO 63026: On and before November 5, 2014, the daily examination for the Mitsubishi Model FGC40K forklift, marked CD001, did not sufficiently recognize adverse conditions which affected the safety of the vehicle. The 7 foot boom was not positively secured to the forklift. This condition exposed employees to struck-by hazards.   b) Control Devices, LLC @ 1555 Larkin Williams Road, Fenton, MO 63026: On and before November 5, 2014, the daily examination for the Mitsubishi Model FGC40K forklift, marked CD001, did not sufficiently recognize adverse conditions which affected the safety of the vehicle. The load rating for the 7 foot boom was not known. This condition exposed employees to struck-by hazards.

29 CFR 1910.184(i)(1): Synthetic web sling(s) were not marked or coded to show rated capacities for each type of hitch and type of synthetic web material:    a) Control Devices, LLC @ 1555 Larkin Williams Road, Fenton, MO 63026: On and before November 5, 2014,  employees in the machine shop area used 2 inch by 5 foot synthetic slings to lift and position hydromat motors during machine set-up and configuration changes. The slings were no longer equipped with load rating tags.
