29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:   a) Bag Department (Mars Volta Bag Machine Area) - On November 5, 2014, Aargus Plastics, Inc. did not ensure the portable fire extinguisher mounted on the yellow column, which was blocked by a pedestal fan and blue table, was readily accessible.   b) Bag Department (Rolls Royce Bag Machine Area) - On November 5, 2014, Aargus Plastics, Inc. did not ensure the portable fire extinguisher mounted on the yellow column, which was blocked by a filing cabinet and pedestal fan, was readily accessible.  c) Extrusion Area (Line 26) - On November 5, 2014, Aargus Plastics, Inc. did not ensure the portable fire extinguisher on the floor near the hopper was mounted.    No abatement certification or documentation is required for this item.

29 CFR 1910.157(g)(4): Employees who have been designated to use fire fighting equipment as a part of the emergency action plan were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter:   a) Facility Wide - On November 5, 2014, Aargus Plastics, Inc. did not ensure employees designated to use fire extinguishers were provided training at least annually.   In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
