29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) On November 5, 2014 employees performing roofing activities on a 6 in 12 and 12 in 12 pitched roof did not have their fall arrest harnesses attached to the fall protection system that was in place.  The employees were exposed to a 17 and a half foot fall hazard to a sidewalk or grass.     Western Roofing, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 106543, citation number 1, item number 2, and was affirmed as a final order on November 7, 2011, with respect to a workplace located at Lot 5, Blk 1 Cabella Creek, Meridian, ID 83680.  Western Roofing, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 518418, citation number 1, item number 1, and was affirmed as a final order on August 9, 2012, with respect to a workplace located at East Red Rock Street and North Weston Ave, Meridian, ID 83642.    Western Roofing, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 602299, citation number 1, item number 2, and was affirmed as a final order on December 14, 2012, with respect to a workplace located at 14105 Santa Cruz Street, Caldwell, ID 83607.
