29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Athens Kidney Center, PC dba Commerce Kidney Center - The Biomed Tech poured Peracidin, containing acetic and peroxyacetic acid, to disinfect lines. Chemical resistant goggles were not provided and used, on or about 11/7/14. b) Athens Kidney Center, PC dba Commerce Kidney Center - The Patient Care Techs handled Commercial Solutions Ultra Clorox Germicidal Beach to disinfect equipment. Chemical resistant goggles were not provided and used, on or about 11/7/14.

29 CFR 1910.1200(h)(1): Employees were not provided information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work areas at the time of their initial assignment and whenever a new hazard was introduced into their work area: Athens Kidney Center, PC dba Commerce Kidney Center - Employees handled chemicals such as, but not limited to, Paracidin and bleach. Information and training as required by the hazard communication program had not been conducted, on or about 11/7/14.
