29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.  Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  a)  In the warehouse, the employer did not have the manufacturer's prior written approval to add the yellow top hat,  blue wagon,  yellow platform, and the orange pumpkin cart attachments to the Toyota Forklift #20, Model #6BPU15.  b)  In the warehouse, the employer did not have the manufacturer's prior written approval to add the yellow top hat and blue wagon, yellow platform, and the orange pumpkin cart attachments to the Toyota Forklift #23, Model#6BPU15.   c)  In the warehouse, the employer did not have the manufacturer's prior written approval to add the yellow top hat and blue wagon, yellow platform, and the orange pumpkin cart attachments to the Toyota Forklift #25, Model#6BPU15.   d)  In the warehouse, the employer did not have the manufacturer's prior written approval to add the yellow top hat,  blue wagon,  yellow platform, and the orange pumpkin cart attachments to the Raymond Forklift#17, Model#EASI-OPC30TT.   e)  In the warehouse, the employer did not have the manufacturer's prior written approval to change the capacity, operation, and maintenance instruction plates, tags, or decals.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a)  In the warehouse, the employer did not appropriately mark with plates, tags, or decals the addition of the yellow top hat and blue wagon attachments to the Toyota Forklift #20, Model#6BPU15 to show the change of the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered.   b)  In the warehouse, the employer did not appropriately mark with plates, tags, or decals the addition of the to show the yellow top hat, blue wagon, yellow platform, and the orange pumpkin cart attachments to the Toyota Forklift #25, Model #6BPU15 to show the change of the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered.  c)  In the warehouse, the employer did not appropriately mark with plates, tags, or decals the addition of the yellow top hat, blue wagon, yellow platform, and the orange pumpkin cart attachments to the Raymond Forklift #17, Model#EASI-OPC30TT to show the change of the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered.

29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed:  On November 6, 2014 and prior to this date, the employer did not ensure that forklift operators maneuvering forklift equipment at cross intersections and in obstructed view aisles ways in the warehouse department were sounding their horn creating the potential for forklift collisions.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:    a)  On September 3, 2014 and prior to this date, employees working in the warehouse at W6545 Quality Dr.,  Greenville, WI 54942, and who utilized the order picker equipment that carried a yellow top hat and the blue wagon attachment were exposed to being struck by the loads and equipment during vertical and horizontal lifts. The netting fastened to the top hat attachment created a snagging hazard to the rack system and other store loads causing the top hat equipment to become displaced and fall off to the ground.
