(a) Employees are exposed to fall hazards while using the sheet lifter to move pallets of aluminum sheet goods to and from stacks in the warehouse area. Employees are exposed to falls up to approximately 11 feet.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by nip and shear points:    (a) Cincinnati Shear (10 foot bed) The installed guard was inadequate. The guard was not properly adjusted allowing a 1/2 inch opening exposing the operator(s) to crushing and laceration hazards from the hold down feet and the shear blade.    (b) Cincinnati Shear (80 inch bed) The installed guard was inadequate. The guard was not properly adjusted allowing a 3/8 inch opening exposing the operator(s) to crushing and lacerations hazards from the hold down feet.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    (a) Niagara Brake Press, 1B36-6-8, used with a punch die, the guard was inadequate, the opening was approximately 3 inches allowing access to the danger area.     (b) Verson 206 Brake Press, S/N 5591 used with punch dies, the guard was inadequate, the opening was approximately 3 inches allowing access to the danger area.
