The guard was not properly adjusted allowing a 1/2 inch opening exposing the operator(s) to crushing and laceration hazards from the hold down feet and the shear blade. (b) Cincinnati Shear (80 inch bed) The installed guard was inadequate. The guard was not properly adjusted allowing a 3/8 inch opening exposing the operator(s) to crushing and lacerations hazards from the hold down feet.
