The employee was exposed to the hazard of being crushed by collapsing soil.

29 CFR 1926.651(i)(1): Where the stability of adjoining buildings, walls, or other structures was endangered by excavation operations, support systems such as shoring, bracing or underpinning was not provided to ensure the stability of such structures for the protection of employees: On or about November 7, 2014, at an excavation site located at 8314 E. 71st Street in Tulsa, OK, an employee was working in an excavation where soil was removed from around an electrical pole that was not secured, braced or pinned to ensure stability. The employee was exposed to the hazard of being struck by the falling pole.
