29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) The employer did not develop/establish an energy control program consisting of energy control procedures, implement employee training and periodic inspections with respect to the use of lockout in preventing release of unexpected energy such as during the servicing and maintenance including but not limited to retrieving balls or resetting pins for the AMF model #82-70 pin setting machines (32) of them, to protect employees exposed to moving and energized parts.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a) On or about November 7, 2014 while recreating an incident for a news reporter how an injury could occur when you have a jams in the pin setter was not protected from the rotating 18 inch shaft above his head and the 3 inch shaft on the pulley in the middle section, on the AMF #82-70 pinsetters, while retrieving bowling balls and/or retrieving pins.     b) Guarding was not in place over the 18 inch shaft above head and the 3 inch shaft on the pulley in the middle section on the (32) thirty two lanes on the AMF #8270 pin setters exposing employees to rotating parts.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    a) Guarding was not attached over the pinch point created from the rotating belt pulleys located in the rear of the bowling alley, employees retrieving bowling balls and/or resetting pins were exposed to unguarded pulleys driving the ball return mechanisms movement on the AMF #8270 (32) lanes, pin setters front section at the wooden plank step.    b) Guarding was not attached over the pinch point created from the rotating belt pulleys located in the rear of the bowling alley, employees retrieving bowling balls and/or resetting pins were exposed to unguarded pulleys top section of the tumbler,  driving the ball return mechanisms movement on the AMF #8270  (32) lanes, on the pin setters top portion at the electrical box platform.

29 CFR 1910.219(e)(3)(i): Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.    a) On or about November 7, 2014 while recreating an incident for a news reporter, guarding was not in place for the pin setter tumbler belt exposing employees to pinch points. The inclined belt is part of the ball return drive system, employees were exposed while reaching inside of  the pin setting machine AMF Model #82-70.    b) Guarding was not in place for the 32 lanes of the AMF #8270 pin setters machines tumbler belts front section exposing employees to pinch points.    c) Guarding was not in place for the 32 lanes of the  AMF #8270 pin setters machine tumbler belts top section exposing employees to pinch points.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidentalcontact by use of approved cabinets or other forms of approved enclosures or by any of the following means:    a) The AMF #8270 pin setters electrical drive motors wiring operating at greater than 50 volts were not covered and/or enclosed within the motor on lanes 16, 17 and 18 in that the cover places were missing, exposing employees to live electrical contacts.    b) The AMF #8270 pin setters main electrical control boxes operating at greater than 50 volts were not covered and/or enclosed within the boxes on all 32 lanes exposing employees to live electrical contacts.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.      a) Knockouts on the AMF #8270 pin setters control panels were not enclosed  to prevent employees exposure, in that the panel had opening due to missing knockouts.
