29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating: a) In Molding department - The holding latch used to secure the propane gas cylinder on the Linde forklift with Model number H30T and Serial number 5030 was missing, leaving the propane cylinder unsecured. Employees were exposed to struck-by hazard. b) In Refinishing department - The holding latch used to secure the propane gas cylinder on the Linde forklift with Model number H30T and Serial number 4912 was damaged, leaving the propane cylinder unsecured. Employees were exposed to struck-by hazard.

29 CFR 1910.1000(c): An employee(s) was exposed to Silica in excess of the 8-hour time weighted average limits listed in Table Z-3: a) In DeGate/Shake-out department - An employee was exposed to silica in excess of the calculated 8-hour time weighted average (TWA) concentration. The employee was exposed to respirable crystalline silica at a TWA of 0.720 mg/m3, exceeding the permissible exposure level (PEL) of 0.667 mg/m3. The exposure was derived from a sample collected over a 415 minute sampling period on November 18, 2014. A zero exposure was calculated for remaining un-sampled period of 65 minutes.

An employee working in the DeGate/Shake-out area was exposed to silica above the PEL when he was separating parts from the molding pattern to be processed.

In Melting department, the employee was wearing the N-95 dust mask improperly when sweeping the floor.
