29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area (Construction reference 1926.59):       Employees were not given hazard communication training in the hazards of the chemicals on the site including, but not limited to, asbestos; Jendco heavy duty adhesive spray which contains the hazardous chemicals acetone, propane, hexane and isobutane; gasoline and diesel fuel.

29 CFR 1926.405(b)(1):  Unused openings in cabinets, boxes, and fittings were not effectively closed:      In the office area there was a temporary electrical panel used to supply power to the work area.  There was a hole in the side of one of the boxes mounted on this panel because a "knock-out" was missing.

29 CFR 1926.1101(f)(2)(i):  The employer failed to ensure that the exposure assessment was conducted in time to comply with requirements which are triggered by the exposure data:       Work at the site began in early October 2014. Samples were not submitted to the laboratory for analysis in a timely manner to assure that additional  engineering controls and work practices could be instituted as necessary when permissible exposure limits were exceeded.       a)  Samples were collected on 10/16, 10/20, 10/21, 10/27 and 10/28; these samples were not received by the laboratory until 11/10/14.       b)  Samples collected on 11/3 and 11/4 were not received by the laboratory until 11/10/14.

29 CFR 1926.1101(g)(1)(i):  The employer did not use engineering controls and work practices in all operations covered by this section, regardless of the levels of exposure, in the form of vacuum cleaners equipped with HEPA filters to collect all debris and dust containing ACM and PACM:      Ordinary shop vacuum cleaners were used to collect debris which contained asbestos fibers.
