29 CFR 1910.68(e)(1): Frequency. All manlifts shall be inspected by a competent designated person at intervals of not more than 30 days. Limit switches shall be checked weekly. Manlifts found to be unsafe shall not be operated until properly repaired. The employer did not designate a competent person to conduct monthly inspections of the Humphrey Belt Manlift due to the fact that on or about October 25, 2014, a miller employee fell off a belt manlift from almost the third level to the first level when the step platform fastening, due to wear and tear and metal-on-metal contact to the rail and step, caused the step platform to fall off the belt and to the first level.

29 CFR 1910.68(c)(3)(i): Minimum depth. Steps or platforms shall be not less than 12 inches nor more than 14 inches deep, measured from the belt to the edge of the step or platform. On the Humphrey Belt Manlift, Serial Number 375293 located inside the mill, one of the five step platform depth width measured 9 1/2 inches.
