29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Production area lines one and two: A conveyor belt that fed doughnuts to the proofer had an opening between the flights exposing employees to rotating parts. An employee's hand was crushed and bruised when it got stuck between the opening and the rotating parts, on or about 8/22/14.    b) Production area lines two and three: The chute in the hopper of a dough depositor was not guarded to protect the employees from moving parts. Employees reach into the depositor to push dough down the trough, on or about 11/13/14.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.
