JEB Little Creek Building 3601: On or about 11/14/14, the outer insulation of a green 100' extension cord used with a portable saw was cut, exposing the employee to the internal copper conductor and an electrical shock hazard.    No abatement certification or documentation required.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.    a.)    JEB Little Creek Building 3601, Section (E): On or about 11/14/14, holes in floors that were greater than two inches in diameter were not provided with covers, as a result employees were exposed to trip hazards.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
