29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a) Centennial Avenue and South Avenue East, Cranford, NJ 07016. Employee was exposed to struck-by injuries when spoils from the excavation were placed at the edge of the excavation that was in excess of 10 foot deep. Incident occurred on or about 11/04/14. Jo-Med Contracting Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.651(j)(2), which was contained in OSHA inspection number 989411, citation number 1, item number 3, and was affirmed as a final order on 09/25/204, with respect to a workplace located at Carlson Place & Pawnee Avenue, Lake Hiawatha, NJ 07034.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible caveins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) Centennial Avenue and South Avenue East, Cranford, NJ 07016. A daily inspection of the excavation was not made by a Competent Person. Excavation has visible deficiencies, such as but not limited to, no protective cave-in system and spoil piles less than 2 feet back from the edge with a depth in excess of 10 feet. Incident Occurred on or about 11/04/14. Jo-Med Contracting Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.651(k)(1), which was contained in OSHA inspection number 989411, citation number 1, item number 4 and was affirmed as a final order on 09/25/2014, with respect to a workplace located at Carlson Place and Pawnee Avenue, Lake Hiawatha, NJ07034.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. Excavation was not made entirely in stable rock and exceeded 5 foot in depth. a) Centennial Avenue and South Avenue East, Cranford, NJ 07016. Employee working in an excavation was exposed to struck-by and crushing injuries in an excavation in excess of 10 foot deep which was not adequately sloped, benched or protected by shield and or shoring. Incident occurred on or about 11/04/14. Jo-Med Contracting Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.652(a)(1), which was contained in OSHA inspection number 989411, citation number 1, item number 5 and was affirmed as a final order on 09/25/2014, with respect to a workplace located at Carlson Place and Pawnee Avenue, Lake Hiawatha, NJ 07034.
