29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 17, 2014, at the above addressed jobsite, employees were measuring and prepping for installing insulation on top of a roof of a commercial building without the use of fall protection, exposing the workers to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 17 November 2014, at the above addressed jobsite, an employee was utilizing a aluminum extension ladder to access the roof without extending the ladder at least 3-feet above the upper landing surface, exposing the employee to a fall hazard.
