29 CFR 1910.268(d)(1): Before work is begun in the vicinity of vehicular or pedestrian traffic which may endanger employees, warning signs and/or flags or other traffic control devices were not placed conspicuously to alert and channel approaching traffic: (a)Intersection of Main St. and Boland Way, Springfield, MA: On or about October 21, 2014, no warning signs, flags or other traffic control devices were conspicuously placed to alert and channel traffic which was approaching from 4 directions before work was begun in the manholes at the center of this 4 way intersection. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.268(o)(1)(i): When covers of manholes or vaults are removed, the opening was not promptly guarded by a railing, temporary cover, or other suitable temporary barrier which is appropriate to prevent an accidental fall through the opening and to protect employees working in the manhole from foreign objects entering the manhole: a) Intersection of Main St. and Boland Way, Springfield, MA: On or about October 21, 2014, employees were working about manhole(s) whose cover had been removed and were not subsequently guarded by any means to prevent employees from falling in the open manhole(s) or prevent foreign objects from entering the manhole(s) and injuring the employee working in those manhole(s).
