Custom Tool & Design, Inc. - On or about November 11, 2014, and at times prior thereto: Employees worked with and around hazardous substances including, but not limited to, Ethyl Alcohol, Methyl Alcohol and Acetone. Safety data sheets for materials including, but not limited to, acetone, alcohol and cleaners and coolants were not maintained at the worksite nor was there a method/procedure in place to ensure MSDSs could be provided to an employee requesting access to the document.
