29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a. Located at the CTDI facility at 6100 Opus Drive in Groveport, Ohio, the employer did not ensure that operators were trained and evaluated on the safe use of reach trucks, stand-up forklifts, and motorized pallet jacks   CTDI was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 312981335, citation number 1, item number 3 and was affirmed as a final order on 10/27/2010 with respect to a workplace located at 1000 Armand Hammer Blvd, Pottstown, PA 19464.  CTDI was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 314230921, citation number 2, item number 1 and was affirmed as a final order on 2/1/2011 with respect to a workplace located at 1305 Goshen Parkway, West Chester, PA 19380.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at CTDI at 6100 Opus Drive in Groveport, Ohio:  a. On the 2014 OSHA 300 log, the employer did not ensure that all required information was completed in column F.      1. Case number 7 did not describe the parts of the body affected.      2. Case number 9 did not describe the injury that occurred.       3. Case number 10 did not describe the injury that occurred.       4. Case number 11 did not describe the injury that occurred.       5. Case number 13 did not describe the injury that occurred.       6. Case number 16 did not describe the injury that occurred.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a. Located at the CTDI facility at 6100 Opus Drive in Groveport, Ohio, the designated exit aisles across from dock doors 18 and 24 in the storage racking area were blocked by pallets of product and equipment.
